Say Aloha to $6M Savings From Care Transition Team

A novel initiative in Hawaii, in which hospital and community pharmacists provided intensive medication management and direct handoffs for patients at high risk for medication-related readmissions, yielded big savings and propelled community pharmacists into a more comprehensive role on the care team.
