Transitions of Care Pharmacists Facilitate Post-Discharge Follow-up

Sheila Hwe, PharmD, a PGY2 transitions of care pharmacy resident at Palomar Health in Escondido, Calif., presented a poster at the ASHP Summer Meetings & Exhibition 2017 in Minneapolis, which looked at the pharmacist's role in bridging the gap between inpatient and outpatient care, trying to help patients overcome the barriers to making their follow-up appointments with the primary care physician.
