PHOENIX—A standardized program for converting warfarin patients to direct oral anticoagulants (DOACs) at a high-volume anticoagulation clinic developed at the height of the COVID-19 pandemic has succeeded in keeping 95% of the converted patients on DOAC therapy, according to a study presented at the 2022 ASHP Summer Meetings and Exhibition (poster 14T).
During the initiative, pharmacists from the Yale New Haven Health Heart and Vascular Center, in Connecticut, created several
