M&M Rounds Bring Med Safety to Forefront

Denver—With several departments at Boston’s Brigham and Women’s Hospital having their own morbidity and mortality (M&M) conferences to review patient cases in which medical errors, adverse outcomes or near misses occurred, the Department of Pharmacy Services decided to participate. Since September 2016, pharmacists at the medical center have held their own morbidity, mortality and process improvement rounds, leading to some policy changes and operational
